What to know before scheduling:

• Both new and established patients can use the button above to schedule.

• Established patients can also schedule through the patient portal - Power2Patient.

• It will ask you to pay a “deposit” at the time of scheduling. This is the full price of the visit you are scheduling, plus the credit card processing fee. The system uses the word “deposit,” but this is actually the full cost.

• Due to existing federal regulations Dr. Mallory cannot see patients in this practice who are covered under medicare or medicaid (Badgercare) during the time of their visit.

• Please note, current regulations also require that you be physically located within the state of Wisconsin at the time of your telemedicine visit. You don’t need to be a Wisconsin resident, but you must be in the state at the time of care.

• For new patients, once you have scheduled, watch your e-mail. You will receive messages to set up your patient portal and sign the on-boarding consent forms. These must be completed prior to your first visit.
